UPMC Somerset Home Health offers comprehensive health care in the comfort of your own home. They provide IV Therapy, Wound Care including Wound VAC Therapy, Medication Management and Teaching, Disease Management Teaching, Psychiatric Nursing, Rehabilitation Services, Personal Care Services and Social Services. UPMC Somerset Home Health is accredited by the Joint Commission. An on-call nurse is available 24 hours a day. They were also named HomeCare Elite® for several years and currently have a 5 star rating on Medicare.gov Home Health Compare.
Home Care Eligibility
Anyone living in Somerset or surrounding counties, who are home bound and requires skilled home health care is eligible. While a physician’s order is necessary to enter into the home health program, referrals are welcome from anyone. There are no income or age restrictions. Patients need not have been hospitalized before receiving home health services.
Referrals
Referrals may be made by your physician, the hospital discharge planner, family members, caregivers, or patients themselves. Call our intake nurse if you have questions, or feel that you or your family member may have need for home health services. Call (814) 443-4663.
The Physician’s Role
All services provided by UPMC Somerset Home Health must be prescribed by your physician. Your physician determines the type of care to be provided. In addition, the UPMC Somerset Home Health program has a Medical Director who provides overall direction for care offered through the program
Payment for Services
Payment for services will be billed through your insurance company or may be paid by the patient. When coverage is not available, arrangement for payment may be made through UPMC Somerset.
